To confirm your eligibility for funding, you must provide copies of the applicable documents as outlined in this application. Copies of these documents will be taken and retained on your file. Please refer to our Privacy Policy for information on the security of your records and (if applicable) supply of these records in accordance with our legal obligations. This form is to be completed and submitted to determine your eligibility for CSQ Funding.
Please note before completing the CSQ Eligibility & Evidence Form, you will need to have ready digital or scanned copies of:
- Your right to work in Australia (e.g., Medicare OR Australian/NZ passport OR visa)
- Your eligibility as a Queensland Worker working in Queensland building and construction industry (e.g., Current and relevant QBCC Licence, Q-Leave statement OR Letter from Employer provided on company letterhead)
- Identification – A copy of your current drivers licence or proof of age card. If neither of these are available, please provide your birth certificate or passport.
Note: Making false or misleading statements will deem you ineligible and liable for full standard payment of course fees.
